Here at DiabesityMD our goal is to equip individuals with the tools needed to successfully manage diabetes and obesity using a team-based, collaborative approach.
Appointments can be requested directly by an individual or via a referral from a primary care provider or another specialist.
SERVICE 1:
Diabetes Management
Treat all forms of diabetes including type 1, type 2, post-transplant and Maturity Onset Diabetes of the Young (MODY) using medications, diet and physical activity interventions.
Prescribe, manage, train and provide interpretation of data from devices such as insulin pumps, continuous glucose monitors and Smart Pens.
Monitor appropriate labs.
Provide health coaching and diabetes self-management education (DSME).
SERVICE 2:
Obesity Management
Provide individualized weight loss plans that incorporate recommendations for diet, physical activity, medications, meal replacement plans and referrals to bariatric surgery and therapy as deemed appropriate.
Provide ongoing management and support for those who previously had bariatric surgery including monitoring appropriate labs post-op.
Body composition analysis (based on geographic location).
Assist with medication recommendations and dose adjustments for co-morbid hypertension and cholesterol disorders.
Order appropriate workup to diagnose and treat co-morbid obstructive sleep apnea.
SERVICE 3:
Culinary and Wellness Coaching
provided virtually or in-person based on geographic location
Wellness Coaches who listen without judgment and work with you longitudinally to become “unstuck”
Individual and group culinary coaching via programs such as teaching kitchens, shopping with the doc and pantry tours
SERVICE 4:
Convenient & Accessible Service
Flexible clinic hours with evening and weekend scheduling options and same or next day appointments.
Services can be provided virtually or via pop-up clinics for individuals, small groups, businesses and organizations. Contact us for additional information.
Synchronous and asynchronous virtual care with remote patient monitoring available.
